If I am not in-network with your insurance plan, you may be eligible for reimbursement through your out-of-network benefits.
Upon request, I can provide a superbill that you may submit to your insurance company. Please note that reimbursement is determined by your insurance carrier and cannot be guaranteed.

Good Faith Estimate
Under the No Surprises Act, healthcare providers are required to provide a Good Faith Estimate of expected costs for clients who choose not to use insurance.

Financial Responsibility
Clients are responsible for all fees associated with services rendered. Insurance benefits are a contract between you and your insurance company and coverage cannot be guaranteed.

Any deductible, copay, coinsurance, denied claim, non-covered service or balance not paid by your insurance carrier is the responsibility of the client.

It is the client's responsibility to verify insurance benefits and notify the practice of any changes to insurance coverage or demographic information.
